Care Coordinator – Bridgeport

Job Description

The Child & Family Guidance Center seeks a Care Coordinator to join an innovative team that coordinates services for children/adolescents with significant mental health issues and their families, utilizing the Wraparound Process.

Responsibilities:

Care Coordinator will Work in partnership with families using the Wraparound Process and team approach to identify strengths and needs of the child/family in a culturally responsive manner. The Care Coordinator is responsible for helping the family to identify supports, create a team and develop a Plan of Care. In addition, the Care Coordinator is in charge of facilitating monthly Child & Family Team Meetings. The Care Coordinator maintains weekly contact with the family and the team to follow up on the family’s identified goals and is responsible for the documentation and management of the family’s record.

Requirements:

  • Must Have Bachelor’s degree in Social Work, Psychology, Sociology or in the Human Services field
  • Experience working with children /youth and their families with complex needs
  • Must have reliable transportation and valid driver’s license
  • Must be fluent in Spanish or Portuguese

Community Support Worker – Bridgeport

Job Description

CFGC has an opening for a Community Support Worker. This position requires utilization of the wraparound process to coordinate family team meetings, as well as planning and coordinating with community providers to assure that families are connected to resources, services and supports in the community.

Director of Development & Communications

Job Description

CFGC has an opening for a Director of Development & Communications. The Director of Development & Communications will be responsible in planning, developing and implementing all aspects of the organization’s fundraising, marketing strategy, communications and public relations activities.  The Director of Development & Communications will collaborate with the Board of Directors and President/CEO to create a multi-year plan which increases both visibility for the organization and revenues to support the strategic direction of the organization.

Qualifications:  BA (required) or MA (a plus) in field such as communications, public relations or marketing; 3-5 years of experience in development; marketing, communication or public relations with demonstrated success; Certified Fund Raising Executive (CFRE) (a plus) or a certificate in Fundraising Management; excellent working knowledge of software applications including Word, Access, Excel and Publisher, as well as Adobe; experience with donor database software preferred; current knowledge and experience working with social media; sincere commitment to work collaboratively with all constituent groups, including staff, administration, and board members; a self-starter that is able to work independently.

EMPS Crisis Clinician – Bridgeport

The Child & Family Guidance Center is seeking a licensed crisis therapist (LCSW, LMFT, LPC or License Eligible) to join our cutting-edge Mobile Crisis program, serving the Greater Norwalk area. You will sharpen your clinical skills while providing immediate crisis intervention to young people and their families. Our crisis clinicians provide assessments in the community, offer short-term follow up stabilization, and link the family to supports and services.

This position offers an attractive salary and benefits package. Members of our crisis team must have their own transportation, a valid Connecticut Driver’s license, and a willingness to be mobile. This position includes some ‘on-call’ responsibilities. Spanish speaking applicants preferred.

Front Desk Receptionist – Bridgeport

Job Description

CFGC has an opening for a Front Desk Receptionist, must have excellent customer service skills and experience working in a busy medical office setting.  Candidate must be detail oriented and able to multi-task while staying efficient; answer telephones as well as perform other clerical duties.  Candidate must also work two nights a week from 1-8pm; rotate once a month on a Saturday and provide backup for other clerical staff when needed.

Qualifications:

  • High School Diploma or Medical Assistant Certification (preferred)
  • Two to three year’s medical front desk experience a must

IFP Case Managers

Job Description

CFGC is seeking two IFP Case Manager’s needed for innovative preservation program working to maintain and/or reunite families.  Bilingual (Spanish-speaking) highly valued, should have prior experience as a team member working with parents and children.

This individual will provide crisis intervention and case management services to families with a wraparound base philosophy.  The case manager will provide an intense emergency- based family safety service in a home-based, family-centered and short-term crisis intervention basis. Duties include assessments, providing parenting skills, including education and training regarding new approaches to and alternative behaviors for family members. Helping families obtain what is needed with resources in the community in order to continue to keep their families safe and together.

Qualifications:

  • Bachelor’s Degree in Social Work or other related field
  • Bilingual/Bicultural preferred
  • 2-3 years of experience dealing with families and children who are at risk of abuse and neglect

NFN Federal Clinical Supervisor

Job Description

The Clinical Supervisor for the Nurturing Families Network Program provides day to day supervision, support and training to three home visitors.  The Clinical Supervisor is responsible for conducting assessments in the home, screening families and providing educational/outreach to the community.  The supervisor is responsible for maintaining fidelity to the NFN/PAT model, confidentiality of family records, facilitating professional development for staff and overseeing caseloads. The Clinical Supervisor maintains accurate records and program data as required by agency and program prepares and submits all required data, including program statistics, outcome and evaluation measures to the Director.

Qualifications:

 

Education:  Master’s Degree in social work or related field

Experience:  Minimum of two (2) years working with 0-5 population in a home setting.  Knowledge of Reflective Supervision and Infant Mental Health highly desirable.

Knowledge/Abilities: Strong organizational and coordination skills to handle varied duties and responsibilities are essential.  Basic computer skills, including MS Word and Excel required.

Bilingual (English/Spanish) skills highly desirable

Must have a valid Connecticut driver’s license and available vehicle to use.  Travel within the State of Connecticut is required on a regular basis.

Must be able to work occasional early evening or Saturday, when required.

Turning Point Regional Adolescent Substance Abuse Program – Licensed Clinician

Job Description

CFGC (the Turning Point Regional Adolescent Substance Abuse Program) has an opening for a licensed clinician (LCSW, LMFT, LPC) to work in a home/community-based substance abuse program with adolescents experiencing substance abuse difficulties, emotional and behavioral problems. Prior experience working with adolescents with substance abuse issues is required. Clinician must be a team player with good organization, communication, and collaboration skills. Ability to be flexible, multi-task and to complete all documentation in a timely manner are required. The individual must be highly motivated to learn, obtain certification, and implement A-CRA (Adolescent Community Reinforcement Approach) therapy. This model emphasizes helping youth reduce or eliminate their substance use by making behavior modifications through involvement with pro-social activities. In addition to having weekly individual and family sessions in the office or in the community (as needed), the clinician will co-facilitate weekly group meetings/activities/field trips. Some travel and work at our Norwalk office as well as some evening hours will be required. LADC licensing and Spanish speaking preferred.
