Community Support Worker

Job Description

CFGC is seeking a full-time Community Support Worker to provide case management services to the families in the Norwalk area. This position requires utilization of the wraparound process to coordinate family team meetings, as well as planning and coordinating with community providers to assure that families are connected to resources, services and supports in the community.

Compliance & Quality Assurance Specialist

Job Description

CFGC is currently seeking a Compliance & Quality Assurance Specialist. The Compliance & QA Specialist will ensure compliance with internal and external program standards, including but not limited to:  state and federal contracts, Medicaid fee for service billing, and HIPAA privacy and security rules. The Compliance & Quality Assurance Specialist will play a critical role in monitoring fee-for service billing by ensuring that electronic documentation is complete, accurate and meets  compliance standards for reimbursement by CT DSS/Medicaid and all other funders. The Compliance & Quality Assurance Specialist will perform quality assurance reviews and coordinate policy, procedure and standardized form development.

Requirements:

  • Two to three years of post-graduate experience in human services or mental health
  • Prior compliance and/or quality improvement experience preferred
  • Knowledge and experience with Medicaid reimbursable services preferred
  • Ability to work independently on multiple tasks while meeting deadlines
  • High level of skill with MS Office software including  Word, Excel, and Outlook
  • Excellent interpersonal and relationship building skills across business groups internally and outside the organization
  • Excellent communication skills, verbally and in writing.

Education: 

MSW, MBA, MPH or related MA/MS degree.

EMPS Program Supervisor – Norwalk

Job Description

EMPS Crisis Intervention Program Supervisor provides crisis assessments and short-term follow-up for children and adolescents experiencing an emotional and/or behavioral health crisis in the community. The Program Supervisor for EMPS will manage general oversight of the program, including schedules, reports and record reviews. The Program Supervisor will provide weekly supervision for the crisis clinicians and interns, including performance evaluations. In addition, the Program Supervisor will also provide mobile clinical assessments and short-term treatment of children/adolescents as indicated. The Program Supervisor will assist with interfacing with the local hospitals, police, schools and community providers to ensure collaboration. The candidate must have knowledge and experience working with electronic records and data monitoring.

Licensed Master’s degree required in the human service field as well as 5 – 10 years of clinical and supervisory experience. Must have transportation and be willing to be mobile.

Family Based Recovery Clinician

Job Description

The Family Based Recovery (FBR) team at The Child and Family Guidance Center is currently seeking a Masters Level Clinician: Parent-child /Substance abuse clinician. The FBR program is designed as an intensive, in-home clinical treatment program for families with young children at risk for abuse and or neglect, poor developmental outcomes and/or removal due to parental substance use. The Parent-child Clinician delivers interventions to facilitate positive parent-child interactions and optimal child development. The substance use clinician delivers interventions that target parental sobriety and psychological well-being. FBR provides clinical services throughout Fairfield County. The clinician needs to have their own transportation with a valid Connecticut driver’s license. Spanish-speaking is preferred.

Family Support Provider

Job Description

 

CFGC is currently seeking a Family Support Provider(formerly Home Visitor) in the Nurturing Families Network. FSP’s work directly with families. They visit families in the their homes weekly, to provide information and education using the Parents As Teachers evidenced-based curriculum to improve parenting skills and teach the parent(s) anticipatory guidance, age appropriate child development and how to advocate and access community resources and needed services.

Requirements:

  • Bachelor’s degree with a minimum two years’ experience working with young children and families, birth to age five
  • Knowledge of child abuse and neglect (mandated reporting) statutes preferred.
  • Excellent communication skills both verbal and written.
  • Computer literacy required (program and agency uses EMR systems for documentation) and knowledge of basic computer skills (Microsoft Outlook, Word and Excel, etc).
  • Ability and willingness to work with families of all races and socio-economic backgrounds with cultural competence and sensitivity.
  • Must have valid CT driver’s license
  • Must have reliable transportation and able to drive to home visits and to attend collaborative meetings and required trainings within CT
  • Must have adequate automobile liability insurance
  • May have to use own vehicle for client transport
  • Must be able to work with clients in a variety of settings and make home visits

IFP Case Manager

Job Description

CFGC is seeking a Full-time IFP Case Manager’s for innovative preservation program working to maintain and/or reunite families. Bilingual (Spanish-speaking) highly valued, should have prior experience as a team member working with parents and children.
This individual will provide crisis intervention and case management services to families with a wraparound base philosophy. The case manager will provide an intense emergency- based family safety service in a home-based, family-centered and short-term crisis intervention basis. Duties include assessments, providing parenting skills, including education and training regarding new approaches to and alternative behaviors for family members. Helping families obtain what is needed with resources in the community in order to continue to keep their families safe and together.

Qualifications:

Multi-Systemic Therapy (MST) Clinician

Job Description

 

CFGC and the MST team are looking to fill a position for a full time clinician.

This is an intensive in home therapeutic position where the clinician carries a small case load but provides services multiple times per week in the family’s home which includes on call responsibilities.

It has non-traditional work hours with on call, evening hours and occasional weekend hours too. The catchment area includes  Fairfield, Bridgeport, Stratford, Monroe, Easton and Trumbull.

Mobility is a must.

Nurturing Connection/Nurturing Group Coordinator

The Nurturing Families Program (State Funded) has an immediate opening for a full-time Nurturing Connections/Nurturing Group Coordinator. The Coordinator is responsible for triaging all intakes, screening families using a standardized tool and promoting and marketing the program. The Coordinator also facilitates Nurturing Parenting Groups at designated times during the year.

Job Summary: 

  • Position calls for a licensed therapist with experience supervising Master’s level clinicians
  • Support vision and mission of the Agency and communicate to staff
  • Provide clinical and administrative support and coverage in the absence of Unit Director
  • Maintain a caseload reflective of the amount of staff supervised in order to meet direct service productivity
  • Responsible for review of staff documentation and weekly supervision
  • Responsible for providing supervision to interns through the agency
  • Conduct annual performance evaluations for assigned staff
  • Provide direct clinical services (individual, family, and/or group therapy) to children, parents and other individuals in the family
  • Provide consultation to other community agencies, schools, courts, and individuals as deemed appropriate
  • Develop a written treatment plan signed by the client and his/her parent or guardian
  • Participate in weekly supervision and attend case conferences and team meetings as assigned to supervise other staff and students and participate in training of other agency personnel

RTFT Clinician

Job Description

CFGC is currently seeking a full-time Master’s level Reunification and Therapeutic Family Time Worker. The Reunification and Therapeutic Family Time worker will provide strength based intensive support for families involved with The Department of Families and Children in the Greater Bridgeport area. The worker conducts regular visits with children and parents in client homes, at our offices or in the community. Workers maintain consistent contact with all involved providers and informal systems of support.

  • The worker will be responsible for providing the following services:
  • Reunification Services which includes parent / child assessment, preparation work prior to reunification; Intensive work following reunification and post reunification work to monitor and provide support to the family;
  • Therapeutic Family Time (TFT), actively working with parent(s) to prepare for a visit and process successes and challenges afterward; during TFT, the worker will focus on parenting skills, improving parent / child interactions and promoting attachments.
  • The worker will be expected to use the Visit Coaching Model when providing TFT, which encourages empathy, empowerment, responsiveness and active parenting.

 

Requirements:

  • Master’s degree in social work, psychology, counseling, or a closely related field and have at least one year of experience working with children and families
  • Must use their own cars to travel to client homes
  • Families with children in foster placement beyond the Greater Fairfield County may require additional travel
  • Must be willing to work flexible hours with periodic weekend and evening visits as necessary
  • Reunification and Therapeutic Family Time workers participate in a cell phone rotation and agree to carry a cell phone for one week, on a rotating basis, providing 24/7 crisis intervention.
