Position Title:  Care Coordinator 

Program Name:  Care Coordination 

Report To: Director of Care Coordination 

Classification: Full time __x___    Part time _____    Exempt _____ Non- Exempt _____  

Supervisory:   Yes _____         No __x___    

Expected Hours of Work and whether overtime is expected.

A flexible and when convenient to the family, thirty-five hour work week is required.  Occasional evening and weekend work may be required as assigned families demand.

BASIC FUNCTIONS:

Care Coordinator is responsible for providing care to children with complex emotional and/or behavioral health challenges using the Wraparound process. He/she works within the guidelines, policies, procedures and mission of the Child and Family Guidance Center and understands and supports the Agency’s strategic plan and its Values. 

ESSENTIAL FUNCTIONS:

Clinical

1. Provide Care Coordination services to 10-12 children/youth with complex emotional and/or behavioral health needs.   

2. Maintains fidelity to the Wraparound process when working with all assigned families. 

3. Develops a crisis plan for each family served that guides the family to appropriate assistance, supports and services in the event of a family mental health crisis.

4. Assists the family with developing and convening monthly Child and Family Team Meetings (CFT) of traditional and non-traditional providers with the purpose of developing a Plan of Care (POC) that is culturally appropriate.

5. Ensures that plans of care and crisis plans are implemented appropriately and adapted by the CFT as needed to meet the multiple and changing needs of the child and family.

6. Provides weekly contact with families served and maintains strength-based, outcome focused documentation of all contacts.

7. Adhere to CFGC guiding principles 

8. Maintains a Wraparound Family Record (WFR) on all children and families receiving Care Coordination Services. 

9. Successfully transitions families out of Care Coordination services by building natural supports, sustainable strategies and pro-active crisis planning.

 

Administrative

1. Gather and submit data required by DCF in a timely manner

2. Assists in program evaluations such as outcome measures and client satisfaction surveys to families and youth (children as appropriate) during service delivery and upon discharge from Care Coordination Service.

3. Participates in monthly care Coordination staff meetings 

4. Participates in monthly Coaching meetings 

5. Utilize supervision to achieve all KPI’s

6. Participate in local and statewide Wraparound trainings

7. Proficient with MS Office software including Word, Excel, and Outlook

8. Ability to multitask and meet deadlines

Collaboration and Teamwork

1. Works in conjunction with community providers such as DCF, Probation, Schools, etc., to ensure seamless, collaborative care coordination for children/youth.

2. Assists in identifying service gaps and in locating resources within the community

3. Represent the agency in a professional and ethical manner at all times 

4. Role models the Wraparound values and principles in all aspects of their work

EDUCATION, CREDENTIAL, EXPERIENCE, and LANGUAGE REQUIRMENTS: 

Education: 

* The position requires a Bachelor’s Degree in Social Work, Sociology, psychology, or in the human service field.  

Credentials:

* Driver’s License (Required)

Experience

* Must possess experience providing direct services to families with complex behavioral health needs.  

* Cultural sensitivity, awareness and knowledge required as well as a strong familiarity of community resources preferred.  

Language

* Language needs are determined based on vacancies and referral trends

 

WORK ENVIRONMENT: 

 This job operates in a professional office environment. This role uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

This role operates in both a professional office environment and in family homes or other locations outside the main office.

 

PHYSICAL DEMANDS: 

In the field-Requires entering family homes, the ability to walk up and down a flight of stairs, occasional exposure to family pets, and use of personal vehicle to travel to various destinations including but not limited to families homes, schools, community agencies, and training sites throughout the State.

In the office- Is a largely sedentary role with shared office spaces.  

Travel

* Travel to and from care coordination families home is required on a regular basis

* Ability to travel to all areas within The Greater Bridgeport area.  

* Travel to training sites throughout the State is expected as needed

EMPS Program Supervisor – Norwalk

Job Description

EMPS Crisis Intervention Program Supervisor provides crisis assessments and short-term follow-up for children and adolescents experiencing an emotional and/or behavioral health crisis in the community. The Program Supervisor for EMPS will manage general oversight of the program, including schedules, reports and record reviews. The Program Supervisor will provide weekly supervision for the crisis clinicians and interns, including performance evaluations. In addition, the Program Supervisor will also provide mobile clinical assessments and short-term treatment of children/adolescents as indicated. The Program Supervisor will assist with interfacing with the local hospitals, police, schools and community providers to ensure collaboration. The candidate must have knowledge and experience working with electronic records and data monitoring.

Licensed Master’s degree required in the human service field as well as 5 – 10 years of clinical and supervisory experience. Must have transportation and be willing to be mobile.

Family Support Provider

Job Description

CFGC is currently seeking a Family Support Provider(formerly Home Visitor) in the Nurturing Families Network. FSP’s work directly with families. They visit families in the their homes weekly, to provide information and education using the Parents As Teachers evidenced-based curriculum to improve parenting skills and teach the parent(s) anticipatory guidance, age appropriate child development and how to advocate and access community resources and needed services.

Requirements:

  • Bachelor’s degree with a minimum two years’ experience working with young children and families, birth to age five
  • Knowledge of child abuse and neglect (mandated reporting) statutes preferred.
  • Excellent communication skills both verbal and written.
  • Computer literacy required (program and agency uses EMR systems for documentation) and knowledge of basic computer skills (Microsoft Outlook, Word and Excel, etc).
  • Ability and willingness to work with families of all races and socio-economic backgrounds with cultural competence and sensitivity.
  • Must have valid CT driver’s license
  • Must have reliable transportation and able to drive to home visits and to attend collaborative meetings and required trainings within CT

Job Summary: 

  • Position calls for a licensed therapist with experience supervising Master’s level clinicians
  • Support vision and mission of the Agency and communicate to staff
  • Provide clinical and administrative support and coverage in the absence of Unit Director
  • Maintain a caseload reflective of the amount of staff supervised in order to meet direct service productivity
  • Responsible for review of staff documentation and weekly supervision
  • Responsible for providing supervision to interns through the agency
  • Conduct annual performance evaluations for assigned staff
  • Provide direct clinical services (individual, family, and/or group therapy) to children, parents and other individuals in the family
  • Provide consultation to other community agencies, schools, courts, and individuals as deemed appropriate
  • Develop a written treatment plan signed by the client and his/her parent or guardian
  • Participate in weekly supervision and attend case conferences and team meetings as assigned to supervise other staff and students and participate in training of other agency personnel

Social Media Intern

Do you love making a difference with words and visual stories? The Child and Family Guidance Center is looking to build its social media presence to increase awareness about childhood mental health and the amazing change our team is making in the lives of children and families in the greater Bridgeport area. The ideal candidate for this internship will have a passion for nonprofit communications, a willingness to learn, along with a balance of both creative and data-oriented thinking.

Reporting to the Director of Development and Communications, the Social Media Intern will:

  • Research and curate content from external “thought leadership” articles to internal stories about the center.
  • Write copy, take photos and create simple graphics for social media posts
  • Increase use of current social media platforms
  • Schedule content ahead of time
  • Collect and organize data using insights, twitter analytics, Hootsuite
  • Download and organize  photos
  • Research nonprofit social media trends
  • Assist in other communications tasks as needed

QUALIFICATIONS

  • A basic familiarity and comfort using social media platforms
  • Creative and professional writing
  • Basic photography skills
  • Excellent judgment and people skills
  • The ideal candidate will have completed coursework that complements all aspects of this position, (ie: communications, marketing, public relations, child psychology, family systems, nonprofit management), but a combination of coursework, experience, and passion to learn will prove beneficial.
  • Timeliness and willing to work on a deadline.

  • Familiarity with data analysis, and basic Excel
