Care Coordinator

Job Description

CFGC is currently looking for a Care Coordinator to join an innovative team utilizing the Wraparound Process to coordinate services for children/adolescents with significant mental health issues and their families.

Responsibilities:

Works in partnership with families using the wraparound process and team approach to identifying strengths and needs of the child/family in a culturally responsive manner. The Care Coordinator is responsible for helping the family to identify supports, create a team and develop a Plan of Care. In addition, the Care Coordinator is in charge of facilitating monthly Child & Family Team Meetings. The Care Coordinator maintains weekly contact with the family and the team to follow up on the family’s identified goals and is responsible for the documentation and management of the family’s record.

Requirements:

  • Must Have Bachelor’s Degree in Social Work, Psychology, Sociology or in the Human Services field
  • Experience working with children / youth and their families with complex needs
  • Must have reliable transportation and valid driver’s license
  • Must be fluent in Spanish

Community Support Worker

Job Description

CFGC is seeking a full-time Community Support Worker to provide case management services to the families in the Norwalk area. This position requires utilization of the wraparound process to coordinate family team meetings, as well as planning and coordinating with community providers to assure that families are connected to resources, services and supports in the community.

Director of Development & Communications

Job Description

The Director of Development & Communications will be responsible to plan, develop and implement all aspects of the organization’s fundraising, marketing strategy, communications and public relations activities. The Director of Development & Communications will collaborate with the Board of Directors and President/CEO to create a multi-year plan which increases both visibility for the organization and revenues to support the strategic direction of the organization.

EMPS Program Supervisor – Bridgeport

Job Description

CFGC is seeking a Mobile Crisis- EMPS Program Supervisor, The Program Supervisor will manage general oversight of the program, including schedules, reports and record reviews. The Program Supervisor will provide weekly supervision for the crisis clinicians and interns, including performance evaluations. In addition, the Program Supervisor will also provide mobile clinical assessments and short-term treatment of children/adolescents as indicated. The Program Supervisor will assist with interfacing with the local hospitals, police, schools and community providers to ensure collaboration. The candidate must have knowledge and experience working with electronic records and data monitoring.

Licensed Master’s degree required in the human service field. The candidate must be capable of providing clinical supervision to Master level staff. Must have transportation and be willing to be mobile.

IFP Case Manager – Full-time and Part-time positions available

Job Description

CFGC is seeking a Part time and a Full-time IFP Case Manager’s for innovative preservation program working to maintain and/or reunite families. Bilingual (Spanish-speaking) highly valued, should have prior experience as a team member working with parents and children.
This individual will provide crisis intervention and case management services to families with a wraparound base philosophy. The case manager will provide an intense emergency- based family safety service in a home-based, family-centered and short-term crisis intervention basis. Duties include assessments, providing parenting skills, including education and training regarding new approaches to and alternative behaviors for family members. Helping families obtain what is needed with resources in the community in order to continue to keep their families safe and together.

Qualifications:

  • Bachelor’s Degree in Social Work or other related field

NFN Federal Clinical Supervisor

Job Description

The Clinical Supervisor for the Nurturing Families Network Program provides day to day supervision, support and training to three home visitors.  The Clinical Supervisor is responsible for conducting assessments in the home, screening families and providing educational/outreach to the community.  The supervisor is responsible for maintaining fidelity to the NFN/PAT model, confidentiality of family records, facilitating professional development for staff and overseeing caseloads. The Clinical Supervisor maintains accurate records and program data as required by agency and program prepares and submits all required data, including program statistics, outcome and evaluation measures to the Director.

Qualifications:

 

Education:  Master’s Degree in social work or related field

Experience:  Minimum of two (2) years working with 0-5 population in a home setting.  Knowledge of Reflective Supervision and Infant Mental Health highly desirable.

Knowledge/Abilities: Strong organizational and coordination skills to handle varied duties and responsibilities are essential.  Basic computer skills, including MS Word and Excel required.

Bilingual (English/Spanish) skills highly desirable

Must have a valid Connecticut driver’s license and available vehicle to use.  Travel within the State of Connecticut is required on a regular basis.

Must be able to work occasional early evening or Saturday, when required.

Outpatient Clinical Supervisor

Job Description

CFGC is seeking a full-time Outpatient Clinical Supervisor in the Outpatient Department.

Job Summary: 

  •    Position calls for a licensed Marriage and Family Therapist with experience supervising Master’s level clinicians
  •    Maintain a caseload reflective of the amount of staff supervised in order to meet direct service productivity
  •    Responsible for review of staff documentation and weekly supervision
  •    Responsible for providing supervision to interns through the agency
  •    Provide direct clinical services (individual, family, and/or group therapy) to children, parents and other individuals in the family;
  •    Provide consultation to other community agencies, schools, courts, and individuals as deemed appropriate;
  •    Develop a written treatment plan signed by the client and his/her parent or guardian;
  •    Participate in weekly supervision and attend case conferences and team meetings as assigned to supervise other staff and  students and participate in training of other agency personnel;
  •    Participate in the development and maintenance of effective working relationships with other community agencies, schools and individuals through the program of clinical services and other activities
  •    Participate on staff committees and in other administrative matters, as assigned;
  •    Participate in research activities of the Center;
  •    Maintain client records up to date.

RTFT Case Worker – P/T

Job Description

Child and Family Guidance Center is searching for a Part Time, 17.5 hours per week, Bachelor’s Level Reunification and Family Time Case Worker to join our team.

The purpose of our Reunification and Therapeutic Family Time Services is to engage, support and intervene with family members through short-term, intensive in-home service following the removal of a child(ren) due to abuse of neglect in order to promote and effect successful reunification and reduce the risk for further abuse or neglect. The Reunification and Therapeutic Family Time worker will provide strength based intensive support for families involved with The Department of Families and Children in the Greater Bridgeport area. The worker conducts regular visits with children and parents in client homes, at our offices or in the community. Workers maintain consistent contact with all involved providers and informal systems of support. The worker will be responsible for providing the following services:

· Reunification Services which includes parent/child assessment
· Preparation work prior to reunification
· Intensive work following reunification and post reunification work to monitor and provide support to the family Therapeutic Family Time (TFT), actively working with parent(s) to prepare for a visit and process successes and challenges afterward. During TFT, the worker will focus on parenting skills, improving parent/ child interactions and promoting attachments. The worker will be expected to use the Visit Coaching Model when providing TFT, which encourages empathy, empowerment, responsiveness and active parenting.

Requirements:

· Bachelor Level Staff will have a Bachelor’s degree in social work, psychology, counseling, or a closely related field
· Must have at least two years of experience working with children and families
· Bilingual is a PLUS
· Must use their own cars to travel to client homes. Families with children in foster placement beyond these cities may require additional travel.
· Must have a valid CT Driver’s License.
· Must be willing to work flexible hours with periodic weekend and evening visits as necessary
· Reunification and Therapeutic Family Time workers participate in a beeper rotation and agree to carry a beeper for one week, on a rotating basis, providing 24/7 crisis intervention.
